if you desire to order online. If you prefer, you may order by check of money order using
snail mail. Click on the link below. Print and fill out the form. Mail completed form with payment
to the address provided at the top of form.

have your order waiting for immediate shipment upon arrival of payment. Once an email order has
been received, it will be held for 2 weeks while waiting for the arrival of payment before being
